For your convenience our office is open from 8:00 am to 5:00 pm Monday through Friday. We are closed in observance of national holidays, New Year, Thanksgiving and Christmas.


After Hours & Emergencies

In the event of an emergency call 911.

We are available when you need us; our physicians can be contacted at any time by calling our office. An Orthopaedic Surgeon will be available at all times to ensure that appropriate care is provided.


Appointments

To ensure services are rendered in a timely manner and efficiently meet the needs of all of our patients, we request scheduled appointments. We realize that it may not be possible to schedule an appointment in every instance, so please contact our office at any time you feel medical care is necessary. We will do everything possible to respond to your needs.

So our physicians can provide the highest level of care possible we request that any x-rays or diagnostic studies (MRIs, CT or EMGs) performed prior to your appointment are available during your visit. The provider who performed these services will be happy to assist you, or you may contact our office for assistance.

To Reschedule or Cancel Appointments
If you need to reschedule or will not be able to keep your appointment we appreciate notification at your earliest convenience. This will allow us to provide prompt service to others in need.


New and Established Patients

As a new patient you will be asked to complete a registration form by providing your address, insurance and other information. Your signature will authorize us to release information about your care to your insurance company. From our established patients we ask that you notify us should any of your information change.

Workers Compensation patients will be asked to provide information about their employer and injury.


Medical Records

We are committed to ensure your privacy by maintaining the confidentiality of your health care and medical information. Our policies and the law require us to obtain a signed authorization before we can release any medical information to other physicians, insurance companies, or to the patients themselves. In most instances a minimal charge is required for reproduction of medical information.


Surgical Procedures

In the event you require surgery, your surgeon will discuss the procedure and any options with you in detail. Our staff will coordinate the activities of your surgeon, with all other providers and facilities necessary for your care.


Prescription Refills

For your convenience, most prescription refill requests can be responded to during office hours. To ensure that you have enough medicine please monitor it carefully, anticipate your needs and call your pharmacy in advance of running out. We recommend contacting your pharmacy at least two days before your supply of medicine is depleted.

Unfortunately we are unable to respond to your prescription requests after hours.
